| The CCNT is an amazing team comprising of nurse specialists, CCNs, HCAs and Admin. The team is very flexible and offers a responsive service to the families of children and young people they care for. The staff involved are all very caring and knowledgeable, they go above and beyond for the families they work with. They help to navigate the families through transition into adult care, packages of care and supporting and working together with many other members of the MDT including those from health, education and social care. The team really makes a difference to the families they work with day in and day out. It is a real pleasure to work alongside these incredible staff members especially during the recent difficult times we have all faced and they have continued to smile throughout and do their job brilliantly. |
