Mel has been working in the community supporting paediatric patients. Recently she has taken on a number of complex patients including those with challenging neurological and cardiac diagnoses.
In particular Mel has supported a complex cardiac child at the end of their life. Mel developed a very close bond with the child and family and went out of her way to support and help them when they needed it most. i know the family were incredibly grateful to Mel for her time, care and assistance. The welfare of this child and extended family were at the centre of Mel’s care.
Mel’s team working skills during this period were excellent – she communicated regularly when needed with the acute medical team, it was her who suggested to refer early to the hospice and involve them not only with end of life planning but also with respite and personal support. She also organised the ACP for the patient to ensure that nothing was left to chance.
Mel is a forward thinker who takes her patients needs into account first and foremost. She is a great advocate for patients and a wonderful team member.
