The Neonatal Outreach Team have worked really hard this year to facilitate safe early discharge for 157 families across the neonatal service line. They work closely with the wider MDT and community services to plan and facilitate safe discharge plans to enable parents to their infants home and receive ongoing specialist nursing care in their own home environment. The team are committed to supporting families to thrive in their home environment, have a positive transition from hospital to home and ultimately improve long term outcomes for infants. The team have been key in enabling the introduction of the home phototherapy service, further development of the bereavement services and supporting educational opportunities to develop the workforce, all of which are benefitting families and supportive of our Fi Care approach.
