Torbay Children’s diabetes team is a small but dedicated MDT who work wonderfully as a team to provide the best service they can to our young people with diabetes. The team are constantly trying to find new ways to improve care, optimise patient/family engagement and embrace new technologies to ensure that our patients are receiving the optimum care. They treat each patient as an individual and work to meet their individual needs whether that is with home visits, school visits, ward admissions, telephone consultations, activity days, group meet ups etc. The team includes the diabetes specialist nurses, Diabetes specialist dietitians, clinical psychologists, health care assistant and paediatricians with a diabetes interest.
The team works closely with the children’s ward for all inpatient management as well as with the transition service and adult diabetes team to ensure seamless transition into adult services.
Torbay children’s diabetes team really is the most wonderful team to work with and be part of, every single member deserves to be nominated in their individual categories but as a team they really excel.
