Jasmine is our Paediatric Liaison and transition nurse, she is absolutely incredible. She is an absolute fountain of knowledge and always on hand to provide advice and guidance for the more complex patients and discharges. When a patient comes in, who might need some additional home support my first question is always….do you know Jasmine!? If not, she is the first person I go to, as I know she will get the ball rolling and know exactly who needs to be involved. Jasmine is always willing to take on the challenge, always putting the patient at the fore front and ensuring that parents and families feel listened to and are communicated with. Jasmine will ensure all the necessary support, relevant healthcare professionals and community are all working together to provide the best possible care. Patients and families love her, as do her colleagues. She is always around for a chat, a cuddle and as a sounding board and will help support staff on the ward to complete training. I have never seen this woman flap, or get stressed and always acts with the utmost professionalism. I don’t know what we, the patients and families would do without you!
