“The NDDH CCN Team can take on any task you give them big or small.

Airway/trache issues? Got you covered! PEG/MicKey/Line problem? Got you covered! Run out of HepSal? Got you covered! Wound needing packed or dressing changed? Got you covered! HSP and GP won’t dip the urine or do the BP? CCNs got you covered! In fact they have it all covered! NDDH CCNs cover a huge geographical area and can make a massive difference to the care we provide for patients and their families by delivering care in their homes, on their doorstep, in their garden and even through their car window (needs must during COVID!). Headed by Neal Carter, the team work amazingly together to coordinate ambulatory care for our large rural population. They help keep kids out of hospital, support them to get home quicker and help reassure & support worried parents. The CCN team is often the 1st port of call for many of our chronic and complex patients and the team provides a valuable link between home and hospital care. The CCNs also take referrals direct from GPs and help take some of the burden away from GPs by sorting out prescriptions, troubleshooting acute problems and carrying out home visits for asthma follow ups. If they can’t keep their patients at home the CCNs will also regularly visit them on the ward and support the nurses and doctors looking after them.
